A missing or invalid medical card was the No. 1 driver violation at CVSA's 2026 International Roadcheck, at 28%, up from 18% the year before (Fleetworthy). Most of these out-of-service orders are avoidable. Use this checklist to stay on the road.
Quick checklist
- Your medical card is valid for at least the next two months, or your exam is booked.
- Your CDL and endorsements are current.
- You're registered in the Clearinghouse, with no consent requests waiting.
- Your ELD is on FMCSA's registered devices list.
- Copies of your medical card and license are in the truck.
Your DOT medical card
- Know your expiration date. A medical certificate is valid for up to 24 months, and often less if you have a condition that needs monitoring.
- Book your exam at least a month early, with an examiner listed on FMCSA's National Registry.
- Confirm your state has your new certificate on file after the exam. Ask your examiner how results are reported.
- Keep a copy available while you drive.
What happens if your medical card lapses?
You can't legally drive a commercial truck without a current medical certificate. An inspector can place you out of service on the spot, and if your state doesn't receive a new certificate, it can downgrade your CDL until you fix it. Your carrier can't dispatch you in the meantime, so a lapsed card costs you miles and pay.
Your CDL
- Check your license and endorsement expiration dates.
- If you hold a non-domiciled CDL, check the new eligibility rules before renewal. Since March 16, 2026, renewals require proof of eligible status (FreightWaves).
- Report traffic convictions to your employer as required.
Drug and Alcohol Clearinghouse
- Register and respond to employer consent requests promptly.
- Know your status. More than 202,000 CDL and permit holders were in prohibited status at the start of 2026 (FreightWaves).
English proficiency
Since June 2025, inspectors can place drivers out of service for failing English language proficiency, and FMCSA has proposed writing that into its rules. Be ready to communicate with officers, read road signs and complete paperwork in English.
Your ELD
- Confirm your device is still on FMCSA's Registered Devices list. FMCSA revoked 15 ELDs in two months during 2026 (Fleetworthy).
Rules change often, so confirm requirements with FMCSA or your safety department. Apply to drive with American Miles.
